selvanr4 Posted March 11, 2013 Posted March 11, 2013 (edited) !9 day old newborn has been brought by the parents for knowing what to do with the multiple haemangiomas present from birth. At present baby is fine on exclusive nreastfeeding. 1) What is the liklehood of this baby having internal haemangiomas? 2)what are the investigations of choice? 3)when should we think about propranolol treatment? 9ermission to post has been obtained from parents) Edited March 11, 2013 by selvanr4
Dinesh N Patel Posted March 13, 2013 Posted March 13, 2013 It looks like pleomorphic maculopapular rash - chicken pox in the picture
Stefan Johansson Posted March 13, 2013 Posted March 13, 2013 I agree with @Dinesh, looks like a varicella rash. If correct you would see crusts coming soon - any such development? For multiple hemangiomas, we "screen" brain and liver for hemangiomas, using ultrasound.
selvanr4 Posted March 13, 2013 Author Posted March 13, 2013 it was noted since birth. no maternal rash and still no crusts! it is haemangioma. Probably my 3.2 megapixel mobile camera is not good in capturing. when i see them for review, will post latest pictures thanks selvan
Stefan Johansson Posted March 13, 2013 Posted March 13, 2013 Just to confirm the diagnosis, would it be possible to get a skin biopsi? Here're 2 reference about propranolol: http://www.ncbi.nlm.nih.gov/pubmed/23429945 http://www.ncbi.nlm.nih.gov/pubmed/23278381 If the baby is just well and there is no signs of hemangiomas on complicating sites (airway/liver/brain), I'd think it would be possible with a conservative approach. The great number of hemangioms might be problematic, especially if they are grow much before they start to go back spontaneously. I guess a pediatric dermatologist might vore for beta-blockade. Do you have a pediatric dermatologist to consult?
