We have no fixed guidelines for erythrocyte transfusions but usually consider to transfuse to infants on mechanical ventilation with suppl oxygen if the EVF is less than 40%. In well preterm infants we rarely transfuse, but if EVF drops below 25% and if the reticulocyte count is not very high, we consider transfusion.
