There is an evidence-based clinical practice guideline published by AWHONN (Association of Women's Health, Obstetric, and Neonatal Nurses) and NANN (National Association of Neonatal Nurses) in 2002. There may be a new publication in the works.

We are looking toward implementing a skin integrity assessment tool. Does anyone have a good one?? we have a nappy score and modified VIP score but need something more generalised to work alongside them.
