a mom and a nurse!!!
Dear netter, We have a neonatal nurse who gave birth to a 23 weeks GA female baby on December, 2008. The nurse finished her maternity leave and joined back last month. She insisted to be assigned on her baby beside taking care of another baby as the ratio is 2:1. The baby now weighs 1680 gm, on full NGT feed, has ROP stage IV and brain US showed dilated ventricles. Otherwise, she is stable. The question is, have you ever encountered such situation where the nurse taking care of the baby, is the mother? Do you think its ethical to deny this mom the right to take care of her baby while she is on duty? Would she be able to take care of the other assigned baby? Would she treat them equally? and if you were in her place, what would you do? Does anyone has any reference about this issue? any input is appreciated

Infants with MRSA infection
We have an infant in our NICU who developed Methicillin-resistant Staphylococcus aureus infection. Infection control ppl asked as to bathe him with hibscrub which contains (4% chlorhexidine, meythlated spirit and perfume) they said it can be dilluted before the procedure. I am concerned about the systemic absorption and irritation that such contents may cause to the infant's skin anybody can share with me their policy for infants with MRSA infections Thanks a lot

bathing sick neonates
We dont bathe sick newborns. They are cleaned with gauze and warm water after stabilization. Even the normal newborn bath is postponded for 4-6 hours until their body temperature is stabilized. The daily bath also is not recommended

Fake nails and nail polish
We dont have a policy for fake nails, but nurses are not allowed to wear nail polish. I dont see it as a problem in our unit because we rarely see ppl wearing fake nails.

Nitric Oxide effect on nurses
We recently had an incident where two of the nurses taking care of a baby on nitric oxide fainted, and one doctor complained of lightheadedness. We had the respiratory therapiest check for any leak but he found the connections are ok and that there is no leak. I wonder if anybody had the same incident and what they did to overcome it. Thanks

PDA treatment - indo vs ibuprofen
We dont use Ibuprofen in our practice. However I have done a seminar about the use of Ibuprofen for the treatment of PDA. all RCT concluded that IBU & INDO have similar efficacy on PDA, but infants treated with IBU experience lower serum creatinine values, higher urine output, and less undesirable decreased organ blood flow and vaso-constrictive adverse effects.
