We would do echo's liberally, because many neonatologist have learned echo and we have our own machine on the ward, i.e. the availability is 24/7.

Our current guidelines (in Stockholm, Karolinska included) is to do an echo no later than the 3rd postnatal day in

  • all infants <27 weeks regardless of respiratory support
  • all preterm infants on needing mechanical ventilation, regardless of gestational age

This is our pragmatic approach but there is some scientific support for it, see this paper in JAMA from last year
