New forum (members-only) - Error reporting and patient safety

Medical errors leading to complications and even deaths, directly or indirectly, occur in all hospitals around the world. NICUs are not immune from such errors. Many errors are because of system faults and can be prevented.

The best way to prevent errors is to learn from our mistakes. Therefore, many NICUs have some sort of reporting system in place in which errors (of omission or commission) are reported.

We have decided to open up a forum dedicated for sharing reports related to errors (including near-misses), which happen in your NICUs. In a true spirit of learning and improving ourselves, we are interested in finding ways of disentangling the possible causes of the error/near-miss and finding ways in preventing similar errors in the future.

Only logged-in members can access the forum. In addition and similar to the virtual NICU, the forum is "invisible" and cannot be read by someone browsing 99nicu.org without logging in.

To prevent medico-legal implications and to encourage reports from our members, the posting of error-reports to this forum will be anonymous. Still, we strongly advice submitting members to get parental consent.

If you want to submit a report regarding an error/near-miss that occurred in your NICU, then send a PM to [JACK]. You can also email the report to info@99nicu.org. Send a description of the actual event along with your analysis of what you think went wrong and how you think this occurrence can be prevented from happening again. We will contact you back to inform you of any further modifications needed before we will post the report on your behalf in the forum, without identifying the member's name who submitted the report. This will ensure anonymity for the error-reports. Only 99nicu members can submit an error-report.

Once the report is published, then members can take part in the discussion, as usual.

Only 99nicu members will have access to the forum, and hence able to read the report and the ensuing discussion.

The most important point to remember in discussions following reports, is that our aim is in identifying ways to prevent the error/near-miss from happening again. We hope that this platform will help all our members in improving their NICUs and ultimately make our NICUs a safer place for our patients.
