I wanted to share an interesting paper from the UK, about the costs of operating a human milk bank. Donor human milk is what most of us prefer before preterm formula, until there is mother's own milk, so the interest in banking seems to be increasing. There is even a European Milk Bank Association. We run a relatively large milk bank in our hospital, serving the four NICUs in Stockholm with donor milk (our NICU + three Karolinska NICUs), I think our milk bank handles a few qubic meters of donor milk (few 1000 liters) per year. The paper below is from the UK and they surveyed all UK Milk Banks (but all did not reply) about the costs. The results? The total a…

Good afternoon 99ers, With a more focussed approach to starting Fortifier, we are encountering the challenge of adding Anti Reflux (AR) (alginate) supplements at the appropriate juncture. Does your Unit add Fort & AR in the same feed or do you work out a feeding regime that keeps these supplements separate? How early (ml/kg/day) are you introducing Fortifier? Thank you for your thoughts/opinions All the very best Al

Hello everyone! I would like to ask about the choice of milk you do for an extremely premature neonate when there is not human milk or mothermilk. Here, in Greece some NICU's give EHF instead and when the baby achieves full enteral feeding they change it to premature formula. Thanx in advance

Hi group; our OB colleagues use Methergine (ergonovine) for treatment or prophylaxis of postpartum hemorrhage. The medication label from the manufacturer recommends to hold breast feeding up to 12 hours after last dose due to unpublished reported side effects in the babies i did a literature search and found only one article published in Pubmed that covers that topic: https://pubmed.ncbi.nlm.nih.gov/27846760/ min this prospective study they found no side effects in breastfed babies Are there any other reference on this topic that you can share? is this something that affects your newborn nursery practice in your hospital? Thanks for any inpu…

I'm seeking advice and insights from others on the following issue: How do you manage a four-week-old premature baby with evolving or established chronic lung disease (CLD) who is still on invasive ventilation or non-invasive ventilation (NIV) and requires a significant amount of FiO2 (>40%)? The baby's weight gain has been poor, necessitating an increase in the total fluid intake (TFI) up to 180ml/kg/day of Mom Own Milk (MOM) and fortifiers, as well as the use of diuretics. In my experience, and based on most cases of CLD, these infants benefit from fluid restriction (<150ml/kg/day), which typically leads to a reduction in oxygen requirements. I usua…

For a multi-centre multi-country RCT on formula feeding for healthy term-born neonates (at risk for obesity), we are looking for researchers in Europe who have access to both PeaPod AND BodPod (Air Displacement Plethysmography (ADP)) in their institution. If you do have a PeaPod AND BodPod available, please contact us for potential participation in our trial! Thanks! Best wishes, Jacqueline Muts and Chris van den Akker (also on behalf of Hans van Goudoever) j.muts@amsterdamumc.nl and/or c.h.vandenakker@amsterdamumc.nl

I'm curious what preterm (or infant) growth charts are in typical use in your part of the world? I'm a neonatologist in the U.S. and my personal observation is that local U.S. East coast practice seems to use primarily the Fenton 2013 and Olsen 2010 intrauterine growth charts as targets for extrauterine growth. (For infants and children, it's a mixture of WHO and U.S. CDC charts.) I maintain a set of freely accessible web-based growth chart calculators (PediTools) which have been generally popular (~600,000+ page views per month) but have noticed that my primary traffic is from North America. This leads me to wonder whether other parts of the world use diff…

Hi guys, good afternoon! Today, at Twitter, Nick Embleton brought a discussion about tongue-tie (TT) and breastfeeding (BF). I've been thinking about it for some time now, since I'm very worried about overdiagnose of Ankyloglossia here, in Brazil. I'd like to know what's the tongue tie policie at your facilitys. 1) do you evaluate every baby for TT or just those who have dificult in BF? A: in Brazil, TT evaluation is one of the mandatory triage before discharge from the "well baby nursery" 2) who performes the evaluation? A: in my hospital, both the pediatrician and the speech therapist (ST). Usually, ST's evaluation finds higher incidence of Anky…
