Hello everyone! I would like to ask about the choice of milk you do for an extremely premature neonate when there is not human milk or mothermilk. Here, in Greece some NICU's give EHF instead and when the baby achieves full enteral feeding they change it to premature formula. Thanx in advance

Hi group; our OB colleagues use Methergine (ergonovine) for treatment or prophylaxis of postpartum hemorrhage. I'm seeking advice and insights from others on the following issue: How do you manage a four-week-old premature baby with evolving or established chronic lung disease (CLD) who is still on invasive ventilation or non-invasive ventilation (NIV) and requires a significant amount of FiO2 (>40%)? The baby's weight gain has been poor, necessitating an increase in the total fluid intake (TFI) up to 180ml/kg/day of Mom Own Milk (MOM) and fortifiers, as well as the use of diuretics. I'm curious what preterm (or infant) growth charts are in typical use in your part of the world? I'm a neonatologist in the U.S. and my personal observation is that local U.S. East coast practice seems to use primarily the Fenton 2013 and Olsen 2010 intrauterine growth charts as targets for extrauterine growth. (For infants and children, it's a mixture of WHO and U.S. CDC charts.) I've been thinking about it for some time now, since I'm very worried about overdiagnose of Ankyloglossia here, in Brazil. Hi! I'd like to know what is your experience in enteral feeding advance in preterm with IUGR or centralization? It's well known that a faster incremention in enteral volumes provides faster achievement of full volume without worst outcomes (https://www.nejm.org/doi/full/10.1056/NEJMoa1816654). But, in preterm with IUGR it's very frequent feeding intolerance, even with MOM or DM. Do you use a diferent strategy? Thank you for your attention!

May I ask whether anyone has experience with a prolonged hanging time of a parenteral nutrition (PN) bag (incl lipids) of up to 48h? We are probably changing our PN regimen into an all-in-one bag. Since the bag contains >400 mL, it would suffice for most premature infants for 2 days. One strategy could thus be to prolong hang time from 24h to 48h to cut PN costs by half.
