A male baby normal vaginal Delivery cried spontaneously with satisfactory Apgar, 1,5 and 10 minutes. Mother didn't receive any narcotics at delivery.history of rupture membrane 24 hours before delivery. Amniotic fluid is clear. Baby has good color but no cry. what you think?

While preparing a lecture on the normal transition from intra- to extrauterine life, I also choosed to include a slide on cord-clamping, and the timing of it. As the umbilical vein blood contributes significantly to the systemic cardiac output (through shunting of umbilical vein blood through foramen ovale), there is a physiological reason to clamp/cut the cord once breathing is initiated. Dear Colleagues, We had published the first quality improvement exercise called Project NIK (aimed at standardising the intubation process in neonates) 8 weeks agao. This includes how to standardise premedications and human factors in elective intubation. We have now developed the 2nd quality improvement exercise called Difficult Neonatal Airway management which looks at standardisation of teams, approach when you encounter difficulties with face mask ventilation, intubation or the placement of the airway adjuncts in neonates. I have heard many speakers about the resuscitation trolly on which baby can be resuscitated while cord is not clamped . I want to know what is the status as of now, is it being in development phase, which company is making it and has some research /data come on this. Also is umbilical cord milking feasible or advisable when the baby is born non vigorous and practically ( in terms of amount of blood transferred /time required ) what is the difference between the two methods

Hello, I am paediatric trainee currently working in Level 2 NICU in UK. I am doing the journal club presentation about the use of LMA for administration of surfactant in preterm babies. During my previous placements in Level 3 NiCUs, I never seen anyone using LMAs and I was wondering what experience do the rest of you have with using LMAs in neonates. What training did you undergo? Yesterday I was conducting code in NICU and one fellow was assigned to chest compression and other was providing PPV via ETT. but they were not coordinating in 3:1 ratio. He argued that once ETT inserted then coordination is not required, which was new to me. He based his logic on PALS where coordination between chest compression and PPV is not required. Can someone further elaborate this point, what is your practice in your unit, do you do coordinating chest compression? and also when to switch to PALS in NICU at what gestational age. As far as I know, recent NRP 7th edition tells us chest compression to PPV via ETT ratio is 3:1. Thanks Naveed
