I would usually try inhaled steroids first…have had pretty good success. If that didn’t work then a short course of prednisone, similar to what would be used for asthma. Jody R Gross, MD

bumetanide / AKI
In my experience, doesn’t work any better than furosemide for getting rid of fluid, but electrolyte depletion more severe… JR Gross

Stethescope in NICU
We use littman infant stethoscopes for all babies in the NICU. We also have a stethoscope at each infant's incubator or crib, so as not to cross contaminate the babies.

PT 26 wks with AML
At 30 weeks, not sure chemo is a good idea...in kids with Trisomy 21, disease may totally resolve...why not give it some time? Did the baby have a urethra somewhere and an anus?? Could be a sacrococcygeal teratoma, but those kids usually have identifiable genitals....

Congenital central hypoventilation syndrome (‘Ondine’s curse)
Very important to monitor CO2, both awake and asleep. If pCO2's are higher than mid 50's, you are not ventilating enough, and setting the baby up for pulmonary hypertension. We recently had a baby here..she was sent home at about 4 months of age, trach'ed, on a home vent and with a pulse ox. The home ventilators are not ideal for neonates here, so we couldn't send her home sooner, until she had 'grown into' the volume vent we had available. She is now almost 2, and doing beautifully. It is only recently that she has been able to spend more than a few minutes off of the vent. Diaphragmatic pacing will need to wait until she is much older.
