    I agree with Yinghua, this could be a metabolic acidosis primarly which is over-compensated. How has the blood gases developed since birth? Are there any signs of other metabolic dearrangements (hypoglycemia, lactacidos etc)?
  2. Dear all, the name of our community - 99nicu - reflects our ambition to create a meeting place that is 99 percent dedicated to neonatal issues. In other words; there is another 1 percent... Among our original ideas we discussed whether to open a Lounge, where members could relax and chit-chat about other things, such as nice Tapas bars in Barcelona (recent congress venue for ESPR). What do you think - should we open a Lounge!?
  3. We have used ibuprofen (iv) since 2001 and have good experience. Ibuprofen is not a wonder drug, efficacy is the same as for indo (as shown also in the cochrane report) but there are less renal function impairment, which is one advantage.
  4. Do you mean weaning PEEP or nasal/noninvasive CPAP-pressure? My reply was about the latter!
  5. What does the plain x-ray look like? Do you feed the infant formula or breastmilk?
  6. In our units at Karolinska we use breastmilk (donated or the mother's) for so-called trophic feeding. Usually we start during the first days of life, with 0.5-1.0 ml every 3-4 hour.
  7. We usually go from 4-5 cm, 1 cm at a time, to 2 cm. We have no strict protocol for CPAP weaning, it is more of a trial-and-error procedure which we start when the infant is clinically stable and oxygen needs go down. Usually we go directly from 2 cm/low FiO2 -> low flow oxygen on mask -> nothing.
  8. The practice at our hospital is to give betaped (12mg tvice with at least 24 h intervall, if possible) even if there is preeklampsia or a history of diabetes in the mother to all mothers between GA 23-32 weeks.   9. Dear Mirko, to my opinion there is no general need in doing so, i consider the trauma by unneccesary suction higher than the theoretical benefit of it. You might just get vagal stimulation.
