prolonged hypercapnea in chronic lung disease
From my review to literature the recommendation is not to exceed Pco2 of 55 mm gh ; unless it is a research issue .

SiPAP
in the litterature it is mentioned that the rate of failure of extubatin is less ( ie rate of rintubation is less) in somebody who already needed reintubation with N-CPAP with the use of SIPAP .

We use H2 blocker onlly for treatment and not as prophylactic thanks .

"Morning Care" protocol
sick babies in NICU(level111) are just wiped daily with sterile gause soaked with warm water . Those babies in SCBU (level 111), stable and larger > 1500g usually have daily bath in the early morning (night shift) . usually we change incubator weekly .

we use mainly NGT occasionally we use OGT. we do change it routinly Q72hrs unless there is a reason to be change earlier .

how fequent you do NGT aspirate when you feed preterm babies

I beleive that it is iatrogenic too.

premedication for intubation
we usually don not give medication for itubation of VLBW infants , but we give premedication for larger babies such as midazolam .
