hello everyone,i would like to know if magnesium sulfate can be used routinely in the treatment of PPHN ,and if so what is the recommended dose??? we have no inhaled nitric oxide and we tried oral sildenafil with unsatisfactory results.thanks:)

Hi, Stephan and all the members! In our unit we ran across the case of congenital laryngomalacia, associated with minor brain abnormalities (cariotype 46XX). This baby required intubation immediately after birth because of stridor, then was ventilated for 18 days. Several attempts to extubate the baby within 72 hours after birth failed. Our surgeons performed laryngotracheoscopy and the diagnosis of laryngomalacia was established. They recommended 14-day course of prednisolone 1 mg/kg/day for larynx maturation and then repeated extubation. Now baby is 1 month old, spontaneously breathing through ETT. We tried 2 attempts of extub on nCPAP mode, but severe stridor sign…

This is about a 2 days old baby 36weeks,delivered by C section. Mother with urinary infection. Dx with Pneumonia. Developed respiratory distress,managed with Survanta plus nasal CPAP. X rays with Pneumothorax.Evacuated by needle puncture .(32cc of air ). Persistent distress,intubated and with mechanical ventilation improved. New X rays with 30 % ?? pneumothorax .Attending decided not to place chest tube. Big controversy..I would like to hear your opinion in this case. Regards .

Hi I know that in adult unventilated patients the trauma unit used to lavage the pleural space with lignocaine to achieve better analgesia for patients that required an intercostal drain. Yesterday one of our 880grammers on NCPAP developed a pneumothorax large enough to qualify for a ICD. We were wondering if anybody tried the lignocaine lavage in unventilated prems? Apart from the theoretical danger of being absorbed systemically, it seemed like a good idea to save this unventilated baby some pain with breathing and the consequences eg collapse, apnoea, blood pressure fluctuations??? Comments would be appreciated.

Hello I am working on implementing Nasal Intermittent Positive Pressure Ventilation (NIPPV) in my hospital and would like to hear if anybody out there is using it and what ventilator you are using to accomplish this mode. I have the Servo i and planning to use the Hudson infant nasal CPAP cannula. Can anybody please tell me about their success or failure with it and if anybody has a P & P to use with the servo i, i would really appreciate your help. Thx

We've recently begun using hi flow nasal cannulas with the Fisher-Paykel humidification system. I've noticed the water vapor changes to water droplets at times sending small to moderate sized drops of water into the babies nares. Fisher-Paykel recommends using their invasive mode (temp 37 degrees C) for ETT, CPAP and hi flow nasal cannulas. It's been noted that no droplets seem to form at the noninvasive temp (31 degrees C), but this goes against the manufacturers recommendation so we aren't permitted to decrease the temp. Any suggestions/comments regarding this problem are appreciated. I fear the water, unknown amounts at unknown times, could be harmful.
