We would do this differently - admit the family to the maternity ward as usual, check status (the midwife takes temp, breath frequence and pulse) every 6 hours the first day (at 6, 12 and 18 hours) but not later if the baby is completely well after 24 hours. The baby stays at least 48 hours on the maternity ward. We check CRP and blood counts on liberal indications though.

I would suggest that you start to wirhdraw phenytoin. If the baby is free from seizures, then wean off midazolam. If also phenobarb Should be stopped, depends on the clinical scenario as a whole and What the EEG looks like. We commonly " auto-wean" phenobarb , i.e. Keep the dose as such for 3-6 months, with no weight corrections as the baby grows.

I'd say yes to your question, if blood is easily drawn from the catheter (without much pressure on aspiration).

We don't have written guidelines for this, but in principal we work like this: we re-admit infants discharged from maternity, if they need photo-therapy or feeding support the first days after discharge, if we have a single room for the family and baby. We also have quite a few babies transferred to our "home-care", typically ex-preterms but >34weeks and practising breastfeeding while reducing tube feeding. Sometimes we have backlashes, like poor weight gains or if the parents feel uncomfortable, for any reason, being at home. Then we offer a single room for these families as well, to come back, whenever needed.

Believe it or not, but I am sitting on 11000 meters in a jumbo-jet, on my way to Dubai. I will go on tour to the Arabian Peninsula, and lecture about PDA diagnosis and treatment, 25-29/5. My ambition is to share news and views on ductal problems, from my dual perspective as neonatologist and clinical epidemiologist. It will be my first visit to the region, and I am looking forward to it very much, not only for my wish to go on a guided tour of the Desert . First of all, I hope to meet many of you, as I know that many neonatal staff in the Arabic countries are active members here. And, secondly, I will be able to meet the "99nicu co-driver" Habib, aka JACK, for the first time IRL. I am more than grateful to him for joining in 2007 and helping me to "resuscitate" 99nicu.org in 2008. I will give lectures in Dubai, Saudi Arabia and Kuwait, and I will post details on times and venues as soon as I get them.
