An off shoot from this topic is the question of whether lactate informs the commencement of enteral feeds. Do any other units follow this practice. We don't commence feeds until the lactate falls below 4. Regards Ali

Good evening Everyone, With a growing body of evidence supporting simulation training I would like to know if simulation training takes place on your Neonatal Unit. If so, in what form, on our Unit we have a Sim Baby, and once a month hold a simulation scenario for the medical and nursing team. We have noticed recently the number of term infants that we paralyse that sucumb to migratory consolidation. Regardless of regular position changes and physio, whilst they remain paralysed you can almost guarantee changes in lung fields throughout. Has anyone else seen this in their infants. Best wishes Al

Hi everyone, I am intrigued as to whether other units preferentially use an oral or iv preparation when attempting to wean ventilator dependent infants. Our oral preps result in such small doses as the regime tails off I am now concerned that even the smallest vomit/posset/aspirate could inhibit the efficacy of outcome. Would it be more pragmatic to curtail administration to purely iv. Value any thoughts. Best wishes Alistair

We tend to use Spironolactone 1mg/kg Chlorothiazide 10mg/kg twice a day, furosemide in more acute scenarios Best wishes Alistair

Hi, From time to time the pressure/volume loop displays a figure of 8, when I quiz the clinicians as to what it is we are witnessing they draw a blank. The closest answer I've found is that of flow starvation, but is anyone able to break it down further for me so I can enlighten my colleagues? Many thanks Alistair
