We get into a tight corner when we have a sick septic baby who is going downhill even with good cover. And we get a C&S report of Suction tip or ET Tube pointing to another bug and some other sensitive drug. How can we differentiate colonisation from infection with the bug in these situation? I work in a large NICU but when I am not needed for a shift I will go relieving , often to the pediatric ocology unit. There, when sepsis is being treated , antibiotics will be infused into each lumen of a cvl on a regular basis .The care plan will indicate whether the Gentamycin goes in the blue or yellow lumen at a particular time eg . There is literature to support this practice . If both lumens are not cleared of infection before antibiotics are discontinued , the infection will simply recur . Hello! We are wondering about our vaccination scheme for our preterm babies. That preterm babies have a higher risk to suffer from a invasive infection is a well known problem. To protect this children for example the American Academy of Pediatrics, the Department of Health in GB and the STIKO in Germany recommend to start the vaccination schedule early (for example 8 weeks of live). The vaccinations schemes includes diphtheria, tetanus, acellular pertussis, Haemophilus influenzae type b, hepatitis B, poliovirus, and pneumococcal conjugate vaccines. i am wondering where you put your infants ventilation bags. Our nursing staff puts them inside the isolette or radiant warmer bed. Mostly you can find them hanging around at the end of the bed (feet). Sometimes the dedicated mask is connected to the bag, sometimes it is put in a different place of the same bed. Our bags and masks are changed every 7 days together with the vent. circuits for infection control. The rationale for that practice is that most of our nurses believe, that they can find and handle the bags faster and easier as if they were placed outside the beds, attached or clamped on the side of the bed or isolette where one can easily grab them if needed. Dear Colleagues, Most of us on suspicion of neonatal blood stream bacterial infection initiate antibiotic therapy and stop it if the blood cultures are negative and the baby is well (usually 48 hours), but what there does not seem to be a consensus is as to how long antibiotic therapy should be continued in culture proven blood stream infection (not meningitis or osteomyelitis) or culture negative but clinically suspected or surrogate marker supported blood stream infection.
