We use Curosurf as well here at Medical University Vienna, Austria. 200mg/kg for first dose. I was curious about the user guide. How do you apply Curosurf - all at once or fraction to both sides?

Questions I would ask are: * price (not only the respirator but think also about consumables) * complexity - if you are just starting with ventilators who will handle them, what is their experience etc.) * CPAP yes/no * HFOV or not * should the ventilator be movable (e.g. possible to use for transport) * what size of children do you want to ventilate: how small? older children too? narrow down your choice to two machines - test them on the ward before buying.

Very interesting discussion. I personally prefer dopamine, if echo is available i might think about dobutamine. some at our department start norepinephrine (it leads to the least pulmonary hypertension) and again others believe hydrocortisone as first line is best. What article by Murphy do you mean, Charles Paxson?

Newborn database
Here are my thoughts: * first think of what data you want to have in the database * what do you want to do with it (keep track of your clinical data, research etc.) * how is the database going to be maintained * security / access issues once you have established that, go on to choose your software. there are quite a few options out there, but you definitely want to have somebody who is knowledgeable about the software you choose. You probably want to split database software and software for analysis. I highly recommend this book.

Apgar score for normal birth?
I believe it is all about documentation. Naturally we could just write down "all was ok at resuscitation", but due to the universal usage of the apgar score - the meaning of 3 simple numbers tells me alot more about what the child was like in the first minutes of life.
