Cardiovascular Problems

  1. Hello everyone! I'd love to hear your thoughts on the management of hemodynamically significant patent ductus arteriosus (PDA). This new article and its recommendations have left me with many questions. before the releae of these news recommendation , in our service, we only treat hemodynamically significant PDA within the first 7 days of life, primarily in patients under 28 weeks' gestational age. We know that the efficacy of medical treatment drops significantly after this age. Our approach is to opt for surgery only if the PDA doesn't close after two series of medical treatment, and we try not to delay the surgical intervention too much. I'm very interested in learnin…

  2. There is a growing body of data suggesting that pulse oximetry before discharge from maternity wards can identify complex but yet asymptomatic cardiac malformations. It would be interesting to hear how common such screening has become. Please comment below too! The poll is open until 31 Jan 2012.

  3. Hi, A case of ex micropremmie with IUGR who has severe PPHN and BPD. Following a severe PPHN crisis experts suggested trial of Bosentan in additional to the sildenafil already started. further course of oral steroids also started. Worked well and weaned ventilation over two weeks. I have minimal experience with Bosentan and wondered how much it is being used as an adjunct? Recognising used as a second line agent when things are in extremis. (no response to iNO). Questions such as typical length of courses being used? Successes? issues? no hepatic impairment at present. Thanks!

  5. I just wanted to share a link about inotropes, a blog post on a British FOAMed* web site: https://www.paediatricfoam.com/2017/01/inotropes-made-simple/ Managing circulatory failure with potent cardio- and vasoactive drugs can be a challenge, and it is necessary to understand the pathophysiology of the problem to choose the right set of interventions and drugs. *FOAMed = Free Open Access Medical Education

Do you have any experience using Milrinone in cases of PPHN In my NICU we don't have nitrous oxide and we recently had a case of PPHN I used Dopamine and Dobutamine but after reading this article I am not sure whether that was the optimal therapy. I highly appreciate your input.

  10. I am a neonatologist working in a low-income country in a hospital with 6000 + births a year. The hospital also functions as the only neonatal referral center for a region that has 17,000 births a year. Due to being a public institution the mothers of our newborns are mostly from extremely low-income households thus we have high rates of congenital malformations and premature births with the corresponding elevated rates of morbidity and mortality. We are interested in implementing Point of Care Echocardiagram for monitoring pulmonary pressure, cardiac function and PAD as well as implementing Point of Care Ultrasound for pleural effusions, line placement, and to perform tr…
