  2. We are taught in fellowship and by many publications that barotrauma is a factor for VILI in neonates. As a general rule, I use many pointers in management of a ELBW infant during mehanical ventilation to prevent VILI, inluding optimizing lung volume, volume-target ventilation and permissive hypercapnea. I use HFOV as a rescue option when pt develop worsening respiratory acidosis despite implementing the criteria above, or worsening lung compliance as indicated by requiring increase driving pressure on the vent to meet target tidal volumes.     Happy new year 2012 for all of you and your beloved ones. I know that this could be controversial, sometimes we would have bad blood gases results (or other labs) that will get back within normal limits if we control the same test after warming the baby's heal. I wonder if some of you warm the baby's heal before capillary sampling? if yes, what is the most practical and safe method you would use for that? Any available litterature on this subject?     We have seen an increase in unplanned extubations in our NICU. The manufacturer of the tape we used stopped making it, and we are having a tough time finding a good product. I know unplanned extubations happen for a multitude of reasons, but a decent product would be a start. Does anyone have any ideas?

    Hi all, anyone here who has experience with different equipment for neonatal cooling and can advise us all about the pros and cons of the respective products? In the TOBY-trials Tecotherm equipment was used, but since there are some other manufacturers pushing into the market I am curious about the actual or possible advantages/disadvantages of the different models that will show up during clinical use.
