yes,I use blender(Bird) with F & P bubble CPAP. It works wonderfully.

Dear sageaugie, Bubble CPAP is also one type of nasal CPAP .In our unit we have both Dragger Baby log and F & P bubble CPAP ,both are working equally well,but we preferred bubble CPAP and keep Babylog free for Mechanical ventilation.I have no experence of Phonix Nasal CPAP. Dinesh

I am in favour of giving NS bolus 10ml/kg over 30 minute ,ask for functional echocardiography and cardiac support if required .Repeat ABG after a hour or two.W/F cardic function and renal function if normal.this might help to correct metabolic acidosis.rRespiratory function looks to be good. I want to add 1-there are some study that Omegwin has prentive role for cholastasis if you cycle with intralipids. 2-Start trophic feeding as early possile as NBM for prolong period(>7days)is usualy associated with Cholastasis. the highest s.bilirun level icame accross was 19 mg/dl in a pt with NEC With septisemia and NBM for 15 days.,recover complitely with supportive care and Ursioiol

I agree with you l have come across to cholestatic jaundice in ELBW Infants two times with TPN for _?more than 3wks with NEC. Both received intralipids and NBM for more than 15 days. Both improvement with time , treated with ursodiliol, vitamin suppliment(A D, E, K) and supportive care. We have rulled out other cause of cholestatic jaundice in newborn

We use Conventional ventilation with high cpap, PRBc transfusion to keep hematocrit 40,Platelets if thrombocytopenia, FFP IF DIC PRESENT ,ET suction restricted to keep ET TUBE Open,. Avoid over transfusion, minimal handling, supportive. Care ,.

Dextrose+Electrolytes+Amiovin 10% , Intralipids 10%,20% ,Omegawin 20%

babylog 8000 plus,bear 750 PSV.we r happy with both,but babylog 8000 is more user friendly.

we can add adrenaline &\or hydrocortisone in case adreno cortical insufficiency where there no response to routine management(FLuids+dopamine+dobutamine).

Is there any role of fuctional echocardiography to essess myocardic dysfuction? Dopamine is prefered after volume correction.Dobutamine can be added if there is myocardic dysfuction.Also treat cause.

As abg suggest ,there is hypocarbian(<35mm of hg)& hyperoxia(>60-80mm of hg) both supress resp. drive.what is I time ? I suggest I time .35 sec,reduce Fio2 to .21, reduce rate up to 12 to 15 gradually,monitor ABG closely.this might help .what about CNS status?in sever HIE you may require ventilator support for long time.

in my nicu on walls white colour glazed tiles 2 ft by 2 ft size,on ceeling white colour apoxy paint.

Apoxy paint,which is antifungal,is recomanded for operation theater wall can also be used in NICU wall.
