Professor Keith Barrington is a neonatologist and clinical researcher at Sainte Justine University Health Center in Montréal. He is Professor of Paediatrics at the University of Montréal. His particular research interests are in cardiovascular support, in apnea and its treatment, in the ethics of decision making for high risk newborns, and in anything in clinical care that might affect outcomes. The 22 of May 2005 he had a very preterm baby girl at 24 weeks gestation. This blog is an attempt to help others in the field access the important neonatal research literature without delay.

Something great came on my radar, Plupp.org, a charitable Internet search engine. It works exactly the same as any other search engine, but its profits are donated to charity. Technically, Plupp uses Yahoo.com for web searches. Plupp donates its search revenue (raised through adverts displayed on search pages) to the International support of reducing preterm birth and to supporting the health and wellbeing of premature babies and their families across the Globe. I think this is a great initiative that deserves to get viral, click here to share on Facebook and Twitter Please share in your own networks about Plupp. If you are involved in a national charity for preterm babies and families, you could also contact Plupp for partnerships. -

There is a new ebook out on Respiratory Management of Newborns, available for free by an open-access published, and edited by professor Hesham Abdel-Hady, @hehady who is also a 99nicu Team members since years. In the book, you'll learn multiple new aspects of respiratory management of the newborn. For example, ventilator management of infants with unusually severe bronchopulmonary dysplasia and infants with omphalocele is discussed, as well as positioning of endotracheal tube in extremely low birth weight infants, noninvasive respiratory support, utilization of a protocol-driven respiratory management, and more. Further, the book includes a chapter on noninvasive respiratory function monitoring during chest compression, analyzing the efficacy and quality of chest compression and exhaled carbon dioxide. It also provides an overview on new trends in the management of fetal and transitioning lungs in infants delivered prematurely. We have tried to use it but have not felt comfortable with the interpretation. I browsed Pubmed and I could not find any large diagnostic studies (estimating sens/spec, predictive values etc). Here was one better article: http://www.ncbi.nlm.nih.gov/pubmed/22019570 Would be great to hear more input from others who may use it!

What sort of internet resources do you mean: tools like http://nicutools.org/ or fact-based sites like http://emedicine.medscape.com/ ? Pubmed is a must on http://pubmed.gov , as well as Cochrane http://childhealth.cochrane.org/cochrane-reviews-child-health The ADC Podcasts are great http://adc.bmj.com/ ! I guess there are similar sites in pediatrics like http://99nicu.org but I don't know specifically about such.

@Abdul kasim jaleel ahmed Do you mean that breast milk would be different depending on the infant is a boy / girl? We analyze all mother's milk (and fortify individually) and I have not been aware of differences related to offspring sex. Nothing I have thought about at least.

@gayle omansky Thanks for posting pictures, they are very illustrating, and the whole set up looks clearly better than our "tape bridge" method. Do you use the Tegaderm for all infants regardless of gestational age / BW?

@Aymen Eshene We do not cover the umbilicus at all either. We stitch the catheter with a suture that is resorbable (Vicryl) and make a bridge with tape (like Steristrip) to secure it. I am not aware of research into this area. We do not clean the catheter, just leave it as is for the (few) days we keep it. @gayle omansky The securing technique you use seems very interesting to me - would you be able to share photos?

Our guidelines towards the patient are inform the patient (family) that he/she is an MRSA carrier ensure the patient / family to tell other other health care services about this carriage any clinical infektions are cultured for MRSA and positive cultures are treated according to the resistance pattern the most important part is hygiene control - in Sweden MRSA falls under the "law of contagious diseases" which means that we have a set of rules to follow to avoid spread of MRSA within health care and societal settings. so, in a case like yours, we would not give any treatment, just take the regular precautions.
