Medication & Pharmacology

  1. Dear Colleagues, I wanted to know what pre intubation drugs you are using and the rationale if you have a moment. I am also curious if you have a neonate with a difficult airway that had failed because it was vigorous would you paralyse with sedation and what would you use? Our PICU colleagues are pushing us to switching to Rocuronium more for consistency as they can reverse it with Sugammadex. I worry because of the duration for which Rocuronium takes away the babies drive as well as the fact the safety profile in neonates is not known. We currently use Atropine, Fentanyl and Suxamethonium. Has any one used Sugammadex in neonates?

  2. We have been discussing this on our ward. Should we provide therapy to prevent SRMD ( stress-related mucosal disease) in our neonates that are ventilated, do not get any feeds or are otherwise severly ill? And what therapy do you believe is best? If you know any current evidence please share it!

Hello! I am currently working on initiating a music therapy program in our level III NICU. I would like to hear from any of you that have a program in place and how it is working out. How did you get it started? Do you have a music therapist on staff? Do you use Pacifier Activated Lullibies (PALs)? Is the staff supportive? Any advice you can give me would be helpful.

We are updating our sucrose policy and have a few questions. Is there any evidence to support increased incidence of NEC (necrotizing enterocolitis) in infants less than 32 weeks? If so, could you provide articles? Do you use Sucrose in infants less than 32 weeks? Do you use Sucrose for fussy, agitated babies? If so can you provide evidence to support this practice? Thanks, Elizabeth Curry

  8. Dear colleagues Yesterday I became aware of this forum, and I think it is an excellent medium to announce research in the field of Neonatology. Therefore I would like to invite you to take part in research concerning prolonged or chronic pain in the newborn. In the last decades much work has been done regarding acute and procedural pain. Prolonged or chronic pain still needs exploration. With the aid of your opinion and the Delphi method we aim at describing the concept of chronic pain in the newborn, including a definition and description of etiologic and diagnostic determinants. You can find detailed information about the background and methodology on www.ceepain.o…

When we use Ampicillin & Gentamicin for intravenous therapy it has been our habit to dose with the Ampicillin first because it is readily available in the unit, then start the Gentamicin dose second. The discussion at the moment is whether the Gentamicin should be first and how much of a wait between the two medications. Would you all share your thoughts. Thanks, Gayle
