Once you have resolved atelectasis problems, I'd suggest HFOV with a "low lung volume" strategy to counterbalance PIE development, i.e. reduce pressure as much as possible at the expense of increased FiO2.

I am sitting in a tennis hall, waiting for my son to finish his match. I play with my new phone, nokia e72. Why I did not buy an iphone? Well, i just thought I wanted a physical knob keyboard. Any good? I would say both yes and no. Building quality is superb and it works great as a telephone. But browsing is not a very smooth experience. The screen too small. Will explore the pdf-function later for protocols etc. I am thinking about writing a full review, maybe we could start a series "phones for neonatologists" sent from my precious...

And please do not forget: although intracranial bleedings are usually spontaneous events, some infants have injuries as a result of trauma/abuse. Check for other signs of shaken-baby syndrome (such as retinal bleeds) if localization of cerebral bleeds indicate trauma.

As far as I remember (not working in a level-3 now), we had two devices when iNO was introduced, and the business model was that iNO was charged by the hour of use. There are (were) a clocking device measuring the time the tube was delivering iNO through the device. So, it did not matter (from an economical point of view) to have one device stand-by in the machinery room.

Great guide for a tie knot dummy like myself. In our units we wear short sleeve pyjama-like hospital staff clothes. But I will explore this guide till my next opportunity to dress up.

Infants in the Karolinska units commonly have a peripherally inserted cathether (at least if parenteral nutrition is assumed to last for more than a few days), typically from a cubital vein. Our aim is to reach for a central position for the catheter tip, typically in vena cava superior. I personally have good experience from PICs, but I would argue that a regular peripheral catheters is sometimes better, for example during KNS septicemia when one may want to avoid all central lines.

The Swedish guidelines are to stop resuscitation - if the infant is still in asystole after 15 min - if the infant is still without any breathing after 30 min

My practise, lumbar puncture in suspected sepsis with crp>100, to ensure that there is no meningitis. If there are signs of meningitis, this is most likely to prolong the course of antibitiocs (as compared to sepsis without meningitis.)

This is how I am used to do this: 1. the newborn preterm infant is wrapped in plastic after delivery and transferred to the NICU 2. the infants is placed under the radiant heater (with plastic wrappings) for umbilical catheters 3. then, incubator! When PICC or periph arterial catheters are needed, these are generally inserted while the infant is still in the incubator.

I would just like to leak some 99nicu info... the software that we use for this site has been realized in a new version that looks and behaves quite differently compared to the current version. This means that the outlook of 99nicu will/may change in the future (early next year). More good news... a professional webmaster has volonteered to help us with 99nicu.org.

I would just like to share a unusual collaboration, Sweden-Australia. There's a Swedish radiologist working in Australia. Due to the shortage of pediatric radiologists in Stockholm, this person in Australia is "on call" sometimes, i.e. x-rays images are sent by wire (Internet) to Australia, and we get the response back. It's quite a good idea I think. I mean, night time in Sweden is day time in Australia. I wished I could be doing night shift in the day time. Preferably in Australia!
